VIEW ALLRavenscroft Medical Centre: Join our Patient Participation Group
Dear Patients,
We are reaching out to ask if any of our patients would like to join the Ravenscroft Patient participation group. Some patients have previously expressed their interested or have been involved, but we are unsure if you are still interested in being a part of the group.
The PPG was originally set up to involve patients with the care services we provide at Ravenscroft, we are mindful that we have not have a recent meeting and would like to reinstate this.
All that wish to join the PPG, please kindly email ravenscroftmc@nhs.net with your full name and date of birth and mark the email ‘Attention to Practice Manager – PPG’.
Based on uptake, I will then email those patients to organise a meeting so that we can involve patients with shaping our services at Ravenscroft Medical Centre.
Kind Regards,
Puja Raikundalia
Practice Manager

Whilst this may seem extreme, we have had to implement this to tighten up our processes for GDPR/confidentiality. You will be asked by the reception team to produce ID and failure to do so will result in patients being turned away. A form of ID can be Passport, Driving licence, British Resident Permit Cards. Please note that we can only hand information to you or if there is consent or Lasting power of attorney for health and wellbeing on the records for a relative to collect this on your behalf
